Not legal or licensing advice. This checklist is educational and is not a substitute for official DECAL / KOALA application forms, your licensing consultant, or the current FCCLH rules (Chapter 290-2-3). Rules change. Before you rely on any template

  1. Download or bookmark the current FCCLH Rules and Regulations (290-2-3).
  2. Use DECAL’s official channels (including KOALA) for applications, uploads, and formal notices.
  3. Ask your licensing consultant what they expect to see in your home’s binder or digital folder.
  4. Treat third-party kits (including ours) as editable helpers, not official forms.

Core child & family files

Chapter 290-2-3 includes detailed children’s records requirements (identifying info, emergency contacts, release persons, immunizations, medical authorization, allergies, and more). A practical checklist:

  • Enrollment / admission packet with child identifying information
  • Emergency contacts and authorized release persons
  • Emergency medical treatment authorization
  • Immunization certificate or allowed exemption documentation (on file by the rule’s timing)
  • Allergy and known medical problem documentation
  • Infant feeding plan (under age one) when applicable (see 290-2-3-.10)
  • Safe sleep acknowledgment / practices documentation (see 290-2-3-.19)
  • Parental agreements for transportation, field trips, swimming, or other away activities when used
  • No-liability-insurance notice + parent acknowledgment if applicable
  • Services description / handbook covering required written procedures (discipline, emergencies, medications, exclusion, parental access, infant sleep notice, etc.)

Daily operations logs

  • Arrival / departure documentation (date, name, times, signature or initials)
  • Attendance summary you can reconcile to care days
  • Medication authorization + dose log when medications are given
  • Accident / illness incident descriptions (date, time, conditions, action taken)

Safety, health & emergency

  • Written emergency plan (fire, weather, utility loss, lockdown, serious injury/death scenarios per rule)
  • Posted emergency phone numbers
  • Fire drills monthly; tornado/other drills on the schedule in rule — keep drill logs (retention often multi-year; confirm current rule text)
  • First aid kit contents + manual + universal precautions awareness
  • Vehicle paperwork if you transport (license, restraints, emergency auth in vehicle as required)

Provider / home credentials (organizer list)

  • Orientation / pre-service and ongoing training records (verify current hour totals in the rulebook)
  • Criminal record check / Livescan status for required adults
  • Zoning / occupancy documentation as required for your locality
  • Water / fire / other local inspections as applicable
  • Insurance certificates you maintain for the business

KOALA vs helper templates

KOALA and DECAL-issued forms win. A paperwork pack can mirror Chapter 290-2-3 concepts so your binder is easier to maintain, but it does not replace official application uploads, CRC processes, or immunization certificates from health providers.
